JOB SUMMARY

  • Provide mental health screenings and initial assessments to medically-referred and other clients.
  • Provide short-term, structured, evidence-based psychotherapy as part of an appropriate care plan.
  • Coordinate and collaborate with primary care providers to improve health outcomes.
  • Supports patient self-management of disease and behavior modification interventions.
  • Facilitate referrals to other services (e.g. substance abuse treatment, specialty care and community resources) as needed.
  • Work closely with Consulting Psychiatrist on challenging cases.
  • Provide clinical therapeutic services to patients, including individual and group therapy and coordination of treatment for participants with co-occurring disorders.
  • Provide crisis intervention as needed.
  • Work to meet patient needs effectively and resolve individual barriers through follow-up, advocacy and collaboration with BTAMC staff and other community service providers.
  • Maintain accurate and timely records of activities, case management notes, and services provided to each client.
  • Participate in and/or facilitate team meetings.
  • Provide regular, written reports to the Medical Director (or CEO) and additional reports as assigned.
  • Other duties as assigned to support the mission of the organization.
Education and Experience:
  • Master’s degree in social work, social sciences, counseling, psychology or closely related field as required for licensure.
  • Current independent clinical license to provide behavioral health services in the State of Pennsylvania or license-eligible at the time of hire (Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Marriage and Family Therapist, Licensed Psychologist)
  • Familiarity and comfort working with issues related to homelessness, substance abuse, mental health diagnosis, and history of trauma.
  • Experience in biopsychosocial assessment; individual therapy, group therapy, short-term, evidenced-based counseling, and crisis intervention.
  • Strong interpersonal, written and oral communication skills; proven ability to communicate with diverse audience.
  • Strong administrative and time management skills.
  • Proficiency in Microsoft Office (Word, Excel, Outlook). Experience with electronic health records a plus.
  • Confidence to communicate and outreach to other community health care organizations, personnel and public.
  • Willing to travel between sites and within community.  Comfort working out of multiple office locations as assigned.
  • Ability to pass all required background checks and clearances
